Getting Started: Registration
All new patients are required to complete a registration process before their first appointment. This helps us provide safe, accurate, and personalized care.
Registration can be completed:
- At the hospital during your first visit, or
- In advance by downloading and filling out the required forms (if available)
Required Documents
Please bring the following documents to your first visit:
- A valid photo ID (for adult patients or parents/guardians)
- Patient’s birth certificate or ID (for children, if available)
- Health insurance card (if applicable)
- Referral letter or medical records from a previous healthcare provider (if available)
- Any current medications or prescriptions
Insurance Information
We work with a wide range of insurance providers. If you plan to use health insurance, please ensure your policy is active and bring your insurance card with you.
If you have questions about coverage, payments, or accepted insurance plans, our front desk team will be happy to assist you.
For patients without insurance, payment options and guidance will be provided at the hospital.

What to Expect on Your First Visit
During your first visit:
- You will check in at the front desk
- Our team will review your registration details

- You will have time to ask questions and understand next steps
